Requests by Mail
Download,
complete, and mail the
transcript request
form to:
Office of the
University Registrar

If you are unable to download the transcript request
form, send a letter requesting a transcript. Include the
following information:
- Your name
and current address
- The name
under which you attended Sewanee. For instance, if
you have a maiden name, please include it in your
request.
- Social
Security number
- Date of
birth
- If you are
a graduate of Sewanee, include the year you
graduated. If you did not graduate, include the
years you attended.
- Indicate
the name and specific address where the completed
transcript is to be mailed.
- Provide a
telephone number where you can be contacted during
the business day if there is a question or problem
regarding your transcript request
- Your
current home mailing address
- Your
signature
We
cannot process your order without your signature. A
computer-generated signature is not acceptable. Under
provisions of the Family Education Rights and Privacy
Act of 1974, as amended, The University of the South
cannot honor a transcript request for a student's
academic record from any other individual (including
parents), government agency, corporation, etc. without a
signed release from the student authorizing the release
of his or her transcript to a specific person,
corporation, or agency.
